Registering with the Practice
We register patients on a rotational basis with the other GP Practices at Strathbrock. Please check with us before completing our registration forms to make sure we are registering new patients this month.
We will ask you to complete and sign a registration form and health questionnaire.
You can download our registration forms and bring the completed forms along with you.
To complete your registration, we also need you to supply us with:
- Proof of your address (eg utility bill, rent agreement, employment documents).
If you are on repeat medications, please make an appointment to see our practice nurse for a health check which includes recording your height and weight, blood pressure check, testing a sample of urine.
Non-urgent advice: Notice
Please note that you are not registered with the practice until all forms have been completed and signed and relevant documentation checked.
